Health pathways nt

HealthPathways is a web-based information portal supporting primary care clinicians to plan patient care through primary, community and secondary health care systems within Northern Territory. These pathways are made up of made up of Clinical, Request and Resource pages. Pathways about Silica Exposurewere also pioneered by the Northern Territory team , and the Clinical Excellence Commissionof NSW has expressed interest in potentially adapting the Northern Territory Sepsis in Adultspathway for their community health setting.

Northern Territory HealthPathways is a locally developed clinical pathways website that supports clinical teams, particularly GPs, to help patients navigate complex local referral pathways and manage health conditions.

HealthPathways Northern Territory is now helping overcome the challenges of distance and diversity in this expansive region, supporting improved patient journeys and helping to guide health professionals through the Territory’s complex health system.
